Travel Risk Assessment

If you are travelling abroad, please make sure you contact us in plenty of time to arrange any vaccinations that may be necessary. We are very busy at the surgery and so it is essential that you try to book your appointment with the travel nurse well in advance of your departure date – about 6-8 weeks before you travel is ideal, and possibly more if the itinerary is complex.

Once we have received your completed online form it will be reviewed by the Travel Nurse, who will advise our Reception Team to contact you to arrange the relevant appointment/s. This may be a face-to-face or a telephone consultation.

Please be advised we are only able to administer NHS travel vaccinations, which are: polio (as a combination with diphtheria, tetanus, and polio), typhoid, hepatitis A, and cholera.

Other vaccinations required for travel purposes will need to be arranged by the patient directly via a local travel clinic.
